Travel Vaccinationsglobal air travel

If you require any vaccinations relating to foreign travel you must complete our travel questionnaire.  This can be collected from our reception or can be downloaded here.  This form includes which countries and areas within countries that you are visiting to determine what vaccinations are required. 

Once we have received the completed form, a telephone consultation will be made for you with the practice nurse to discuss your travel arrangement.  Please note that not all vaccinations are covered by the NHS and therefore carry a charge for both the prescription and administering the vaccine.  It is important that you complete the form as early as possible - at least 6 weeks before you travel - as appointments for both the consultation as well as to receive the vaccinations must be made.  These vaccines have to be ordered as they are not a stock vaccine, and also the appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
